Who needs the AF form 25/26?
The AF form 25/26 is the Application for New/Renewal of a Firearms Identification Card or License to Carry Firearms or License to Possess a Machine Gun. The AF 25/26 Application must be submitted by an individual wanting to obtain legal permission to carry firearms.
What is the purpose of AF form 25/26?
The submission of the form is necessary for the applicant to provide all the required information and to get approved for an appointment on the basis of what requested type of license is being requested. There are several types licenses to apply for with the help of AF-25/26 form:

Firearms Identification Card — Restricted (mace and pepper spray)

Class B License to Carry — Non-Large Capacity

Class A License to Carry — Large Capacity

License to Possess a Machine Gun
Is the AF form 25/26 accompanied by any other forms?
There is a list of required attachments that must be submitted to be eligible for a firearm ID card or license to carry; these attachments differ depending on whether the form is being submitted as a new or renewal application.
The applicant should provide the following documents:

the Safety Course Certificate;

two Character Reference Letters (not from family members);

A valid Driver's License (or Massachusetts Identification Card);

Proof of residence.
If the applicant is a minor (between the age 15 and 17) it is necessary to attach a letter from a parent or guardian allowing the issuance of a firearms' identification (FID) card.
If the AF 25/26 form is submitted as a renewal application, there are fewer attachments required. They include:

The most recent FID card or License to Carry Firearms;

A valid Driver’s License;

Two Character Reference Letter;

Proof of residence.
In both cases, if it is the employer who requires the individual to be legally carrying firearms at work, the letter confirming such a need from the employer must be furnished, too.
When is the AF 25/26 form due?
The application form does not have a due date for submission and can be filed when the need arises (but prior to beginning to carry a firearm).
